The Participant and Parent/Guardian hereby grant Mr. A Cares permission to use the Participant’s name, image, likeness, voice, and appearance as captured in photographs, video, audio recordings, or other digital media during camp activities. These materials may be used at the sole discretion of Mr. A Cares for promotional, educational, informational, or marketing purposes in any format now known or later developed, including print, online platforms, and social media. The Participant and Parent/Guardian understand and agree that no compensation will be provided for the use of such materials, waive any right to inspect or approve the final content, and release Mr. A Cares and its representatives from any and all claims arising out of or related to the use of these materials. This Waiver and Release shall be governed by the laws of the State of Arizona, and any disputes shall be resolved exclusively in the courts located in Maricopa County, Arizona. If any part of this Waiver and Release is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. Acknowledgment: I, the undersigned Participant, confirm that I am over the age of eighteen (18) and that I have read, understood, and agree to all terms set forth in this Waiver and Release. I voluntarily sign this agreement and assume full responsibility for my participation (or the participation of the minor named below) in this program.
